Very painful and seems to be multiplying cystic chin acne

Help, I am a 42 yr old female and I have had mild to moderate acne for appx 20 yrs. Large pores, oily skin, I have been using serious skin care line with fairly good results. About a yr ago I developed cystic acne under chin. Big,red painful and never goes away just sometimes dormant. I have IBS,heart arrythmias,occasional panic attacks,allergies,I work outside in the sun. Can you help? Thank you.
